Transcript Request Information

The following information is provided as a guide to the process of ordering a transcript of your UCSC Extension course work.

Note: All credit card orders must include the full credit card number and expiration date. VISA, MasterCard, American Express, Diner's Club and NOVUS (includes Discover Card, BRAVO and Private Issue) are accepted.

  1. Make your request online. No phone orders can be accepted. Allow up to two weeks for processing. Mail requests to: UCSC Extension Records, 10420 Bubb Road, Cupertino, CA 95014.
  2. Provide your full name and address and daytime phone number. You must provide your Social Security number, which will remain confidential.
  3. If different from your current name, please provide your name as it appears on our records.
  4. Indicate whether you want the transcript sent now showing all graded courses to date or held until a specific class or classes have been graded. If requesting a hold for grades, please indicate the specific class(es) to hold for.
  5. If your work was Concurrent Enrollment, please indicate if you wish to have any narrative evaluations sent, if they are on file at UCSC Extension.
  6. Indicate the number of copies you are requesting. Fees: $10 per copy. Payment is by credit card.
  7. Rush transcript service (one working day priority service) may be obtained by adding the $15 service fee. Please note that no processing takes place over weekends or regularly scheduled holidays.
  8. Provide COMPLETE mailing address information, including zip code, for all recipients of the transcript.
  9. No request will be honored without the signature of the student whose record is being requested. For the online form, your signature is assumed because the transcript is being requested from your student portal.
